Southern Tier Priority Healthcare
Southern Tier Priority Healthcare, Inc. is a voluntary partial capitation Medicaid managed care program operating in Chemung, Schuyler, and Steuben counties.
Included is a per member per month capitation payment for primary care and case management services. Approximately 150 providers serve patients in multiple sites throughout Chemung, Schuyler, and Steuben Counties.
A monthly roster system identifies enrolled patients. Payment for those patients is received at the end of each month. There is no billing required of the provider. Encounter data is provided to the plan when patient services are provided.
PATIENT EDUCATION
Wellness education includes:
Distribution of a self health handbook to enrolled clients
Reminders are sent to patients with no provider encounters during the previous year encouraging a well exam
Education regarding the appropriate use of the healthcare network including the following:
Follow up for missed appointments
Follow up for refusal of the client, to work with their providers in following the prescribed treatment plan
Persistent misuse of the healthcare network results in a recommendation to the local Department of Social Services for disenrollment from the program.
